Consensus double reading of mammograms was more effective and less costly than single reading

چکیده
In the analysis by Brown and colleagues that compared the current rnammography reading policy at St. Margaret's Hospital (double reading with consensus required for recall) with 2 alternative hypothetical strategies, the authors showed that the current strategy used is the most cost-effective. 2 elements of this study must he considered when attempting to generalise the conclusions to other settings. First, the costs assigned in this meta-arialysis are likely to vary among institutions and countries. Therefore, it is necessary to apply local charges to see whether the results of this study can be applied locally. The time and costs assigned for a second reading might exceed the threshold values determined by the sensitivity analysis in other settings (1). Similarly, the costs for assessing abnormal results are mammograms by 1 radiologist; 2) nonconsensus double reading of mammograms by 2 radiologists with recall if recommended by either radiologist; and 3) consensus double reading of mammograms with recall if the radiologists agreed; if they disagreed, a decision was made by the senior consultant radiologist or by discussion between the 2 readers (the strategy in place at St. Margaret's Hospital).

متن کاملEffectiveness and cost-effectiveness of double reading of mammograms in breast cancer screening: findings of a systematic review.
There is a lack of direct evidence on the effectiveness of double reading of breast screening mammograms within the context of national screening programmes even though about half of the countries that use mammography screening have implemented double reading. A systematic review was conducted to compare double reading with single reading of mammograms for screening accuracy, patient outcomes a...
متن کاملIs computer aided detection (CAD) cost effective in screening mammography? A model based on the CADET II study
BACKGROUND Single reading with computer aided detection (CAD) is an alternative to double reading for detecting cancer in screening mammograms. The aim of this study is to investigate whether the use of a single reader with CAD is more cost-effective than double reading. METHODS Based on data from the CADET II study, the cost-effectiveness of single reading with CAD versus double reading was ...
متن کاملCost-Effectiveness of Double Reading versus Single Reading of Mammograms in a Breast Cancer Screening Programme
OBJECTIVES The usual practice in breast cancer screening programmes for mammogram interpretation is to perform double reading. However, little is known about its cost-effectiveness in the context of digital mammography. Our purpose was to evaluate the cost-effectiveness of double reading versus single reading of digital mammograms in a population-based breast cancer screening programme. METHO...
متن کاملComputer-aided detection versus independent double reading of masses on mammograms.
PURPOSE To evaluate the use of a computer-aided detection (CAD) system (designed for mammographic mass detection) to help improve mass interpretation and to compare CAD results with independent double-reading results. MATERIALS AND METHODS Screening mammograms from 500 cases were collected; 125 of these cases were screening-detected cancers, and 125 were interval cancers. Previously obtained ...
